Please complete the following application to be considered for a grant by The Mainstay Foundation. Prior to carrying this out, you must have completed the Organisation verification form and received a Mainstay Foundation verification number. Please note, upon completion of the grant request application form and ultimate approval, you are required to complete a final progress report within three months of the date of the grant award.

  • We will need your name to administer the grant/activity/information requested on behalf of your Organisation/yourself
  • Your email address will be used to contact you with notifications and information regarding your request.
  • We will always try to maintain primary communication through email. If however, this is not proving feasible we will use your number provided to contact you.

  • Please include the amount of funding requested and give the most general description of the use of the funds. (250 words)
  • What are the problems that have created the need for your proposed project that will be funded? What research methods have you applied? (600 words)
  • What do you aim to achieve and how do you propose to do this? (750 words)
  • Select one or more areas your project will focus upon, as per its remit
  • Please provide specific goals to each area of focus above mentioned that your project will address. The goals provided will be subject to measure assessments, as will be discussed
  • How will you measure or define fulfillment of each specified goal? Use measures that are clearly related to your goals and that will highlight your project's impacts.
  • Summarise each step of your project’s implementation in chronological order, taking into account when each activity will be undertaken and or completed. Please ensure your timeline remains realistic. Add rows where necessary

Remember that the project's total budget must equal its total funding.         What we need

        The Mainstay Foundation will be what’s known as the ‘Controller’ of the personal data you provide to us. We use your name, address, email and contact number to process activities/services/responses.

        Why we need it

        We need to know your personal data in order to administer any service or activity and relay information to you, as part of our dealings with you as an individual or in grant related matters the organisation you represent.
        You do not need to provide this information. This will however impact upon our dealings with you and we may not be able to provide you with our activities/services/responses.

        What we do with it

        All the personal data we process is processed by our staff in the UK, the information may however be located on servers outside of the UK but within the European Union. We use the service of Xero UK, an accounting service to process personal data of donors and data of individuals (where grants are made to individuals) for record and accounting purposes. We may also share personal data with our accountants, auditors and financial service providers only where necessary in cases relating to grant making activities and donations by donors. We may also share personal data we hold only where the law requires us to do. We take your data seriously and have a rigorous data protection regime in place to ensure the security and effective procession of your personal data.

        How long we keep it

        We ensure that we do not keep personal data for longer than we need for the purpose it was collected for. This means that personal data that we destroy or erase personal data once it is no longer needed.

        What are your rights?

        If at any point you believe the personal data we process and administer on you is incorrect, or for any other reason you can request to see this information and even have it corrected or erased. You also have the right to ask us to stop processing your personal data.
